Save my solution for the last, I hope you get a better one. But about a year ago, Apple Braille Screen Input really messed up, about every dot was wrong. The only way I found to fix it was to reset all settings. Be careful, you don't want the one that erases all your data, just resets all settings. Unfortunately for me, VoiceOver didn't come back on on its own, and I couldn't use ITunes on my computer to turn it on, being that I had to respond on my phone to ITunes on my computer, and Serie was turned off by resetting these settings, so I couldn't turn it on that way, either. It is the only time I ever have had to get sighted assistance to do something with this phone. Others on this list say that their VoiceOver came back on when they reset all settings, mine did not. I hope you find a better solution, but it did fix the problem. I am using Braille Screen Input to write this message.
