If you are using the keyboard, and say you entered the first name, hit enter, you should then be in the last name. If you pause and listen, VoiceOver should say last name editing, or something like that. If you want Siri to call a place, do not put the name in the company field. It seems that Siri ignores that field. Then, when you have your information in, you will need to get to the upper right corner and double tap, or VO spacebar, on the add button, or done button, whatever it is called now. HTH, Richard
