Hello Karuna, I cannot reproduce your findings.
This is to say, everything is working as expected on an Apple Watch Series 4 running Watch OS6 with iPhone 11, running iOS 13.1. When I receive a call, VoiceOver, on the watch displays both the caller ID as well as the answer and decline buttons. I can activate either of these buttons with the expected results.
