There is definitely a bug for many users where Voiceover and, for me at least, all other sounds and vibrations go away after ending some calls. You should write to Apple Accessibility and also report it because even though they are saying that they are investigating the issue with no timeline as to when it will be fixed, the more complaints they hear, the more likely they are to work on it.
As for your statement regarding Steve Jobs I can only say that nobody knows whether anything would be different were he still alive.
