The Glucose monitor your looking for is the DexCom G5 or G6 system. The G6 connects to your iPhone via bluetooth and sends blood sugar readings to a dedicated application on your iPhone every 5 minutes. This app will also send out alerts when your blood sugar is to low/high, raising/dropping to quickly.
