I have never tried to do this. In looking through the app store, and doing a little searching on the web, it sounds somewhat difficult if it can be done. You are supposed to be able to do this with the Pages app, according to one article; but I do not use Pages, so am not sure if that is something one could do using VoiceOver. I only found one app that sounded like it might do that, but the description makes me think it is a purely visual app. No mention of VoiceOver, and it had low ratings anyway and had not been updated for a few years. So, unless someone has a solid way to do this, I think getting sighted help is your best bet.
