Hi Mike,
I gather the purchase didn’t take out the ads from your app?
I think I would start with Apple Accessibility in that if they cannot tell you, 
they will tell you who to contact.
Since apps are in their own little category now I’m not sure who you would go 
through.  I do know that if it is the iTunes Store, there is no phone contact, 
which is why I’d start with accessibility, 877-204-3930

Let us know what happens.
