Speak screen is primarily for non-VoiceOver users in my opinion. It allows the screen to be spoken when you do a two finger swipe down from the top of the screen, i.e. the status bar. When this is on, you can then lock your phone and it will keep reading. Some people find this useful when reading Apple eBooks. Since if you lock your screen when VoiceOver is reading, it stops.
