I used to use Fantastical and loved it until they went to the bloody 
subscription model and changed the interface. 
I'm not sure you can do what you want.
After putting in the calendar event in Fantastical, open the native calendar 
and edit that event and set it as a yearly event as a birthday.
I had deleted Fantastical months ago, and just downloaded it again to see what 
it is like now.
I will be deleting it again.
