Make sure volume is selected under VoiceOver Rotor. Turn the rotor to volume and flick up or down to adjust the VoiceOver volume. Are you also using Audio Description? That is under The main accessibility settings. You cannot control that volume independant of the movie or TV show.
You can also select Audio ducking in the VoiceOver rotor settings so that any audio playing when VoiceOver speaks, the volume is lowered to try to make it easier to hear VoiceOver.
