I was referring to the Overdrive app. I tried Libby a while ago and at least back then, it was a complete no go with VoiceOver. There's another library service called Cloud Library that some library systems partner with. My experience with the desktop app and Jaws is that it's completely inaccessible, but the iPhone app is actually pretty good and completely accessible. Their app even lets you play back at 3X speed.

The ability to have the playback speed more than the current 2X max and when searching for something, to be able to search all the library systems you're a member of at one time. On overdrive.com, this is how it works For those of us who have multiple library cards, it's a big time help. On the app, you can only search one library system at a time. This would of course not be relevant if you're a member to just one library system though. As far as the other stuff, searching, browsing, checking out a title, adding a library system are all accessible with VO.
