Hi Richard, I just got off the phone with Apple Accessibility and they've escalated the issue, taken a screen recording and copied the phones diagnostics, so hopefully they'll find something simple. This is totally weird, it happened and I wasn't even using the phone.

There is no setting like that. Perhaps going through the reset of your settings; but I think I'd call Apple Accessibility first. But you could reset all settings which means you will not lose any data, just all your settings; which can be tedious to have to go through. Maybe apple would have an easier solution.
