It does this occasionally for both my work Exchange account and my personal Outlook.com account, but I can usually authenticate them without any issues. I do however use 2-factor authentication for both and typically do the authentication via the Microsoft Authenticator app.

My resolution is a work-around, not a true fix. I have a primary Outlook email account that I sync to the native mail, calendar and contacts apps on my iPhone SE 2020 running IOS 15.1. I also check this account via outlook on my laptop. I have not changed the password for quite some time. Suddenly tonight I got an error message when I opened my iPhone mail app saying that I needed to re-enter my password for this account and a secondary account that I recently added to my phone. I followed the prompts to go to settings to enter the passwords. The heading said that my account was not authenticated. With both accounts, I attempted to enter my correct password, but it would come back with the same screen saying that my account was not authenticated. I went to outlook.live.com on my laptop to validate that I was using the correct password and was, indeed, successful at logging into my account. I tired one more time on my phone, but still was not able to authenticate. Since the secondary email account is not really needed on my phone, I figured I’d just delete that account and not worry about that. Oddly, once I did that my primary email account suddenly no longer needed to be authenticated and synced again. All this happened within the last 90 minutes, based on the time stamp for the previous email update. I’m glad to have my primary email and such working, but bothered that I was not having success in authenticating. Have any of you been prompted for passwords, even if you didn’t’ change it?
