Depending on the screen reader, if you tab to the link, then bring up the context menu with shift+f10 or the applications key, there should be a save target as, or save link as option.
At least, with Windows that is the way to do it. Though, using Microsoft Ebge it seems that when I hit enter on a link to a file, it downloads it instead of opening it. Probably some setting I changed along the way that I don’t remember. After all, I’m another old goat, 😊.
