Vertical Navigation is optional and if you want it in your rotor you have to
go to "Settings, General, Accessibility, Voiceover and Rotor and select it
as it is not selected by default. It's the very last item in the list of
things you can add to the rotor.

Action is not something you find in the rotor all the time, it's like Typing
Mode which is also in the rotor, but it's only there when you are in an Edit
field. Actions is only there if you are in an app or more specifically an
area of an app where it is supported. Examples would be the listview of your
emails, the main window of Messages, the Favourites tab of the Phone App,
the list view in the Reminders app, The music app where you can delete
artists, songs and albums etc. Provided you aren't moving the rotor to
something else while you are in one of these apps, Actions will be
automatically selected when it can be used. Remember, if, for example, you
want to spell out an email address letter by letter and you move the rotor
to Characters when you are in your Inbox and if you then exit the Mail App,
Actions will not be selected by default the next time you go into mail. You
have to put the rotor back on Actions before you leave the app if you want
to have it automatically selected next time.

I also just noticed one other oddity and that is that if I have Vertical
Navigation in my rotor and turn the rotor to that setting on the home screen
then it stays on that setting if I go into the Mail App which means focus is
not automatically moved to the Action rotor and a single finger flick down
will just move me down to the next message instead of bringing up the delete
option. I tried every rotor setting I had when I was on the home screen:
characters, words, headings, volume, speech rate, vertical navigation and
language. The Action rotor was automatically active when I opened the Mail
App in every case except when the rotor was turned to Vertical Navigation
before going into the Mail App.
