It works only on the watch and is fully accessible. You cannot have the VoiceOver Digital Crown Navigation on while using the app if you want to adjust the beats per minute while VO is on. To toggle the Digital Crown Navigation, perform a two finger triple tap on the watch. With the Digital Crown Navigation off, the app is truly fully accessible. you adjust the BPM with the digital crown, and VoiceOver reports what you've set it to. The watch taps your wrist and also beeps if Silent Mode is off. There's also a start button, which becomes a Stop button when you select it. The metronome isn't accurate on my watch, but I think that's because it's a Series 3. Reviewers said it works well on watches with the Always On feature.
