The first thing to check is to make sure the phone is not muted. The mute or silence switch is a small switch probably on the left edge of the phone near the upper left corner. You change the switch with your fingernail.
Hold the phone flat with the screen facing up. Have the phone unlocked with VoiceOver on. The phone is muted when the switch is in the up position. To unmute the phone, push the switch down. If the phone is unlocked and VoiceOver is on, it will say "unmuted".
