after rebooting the phone, do the 2 finger quadruple tap and you should find it in the quick settings if you left it turned on in VoiceOver Recognition.
Then it might show up in the settings ... this is not one of Apple's better attempts at a feature.

First, Screen Recognition came in with iOS 14 though it was seldom talked about until iOS 15.

 

To have it show up in Quick Settings, you have to first turn it on under Settings, Accessibility, VoiceOver, VoiceOver Recognition.

If it is not on there, it will not be in Quick Settings.

 

I tested this by turning it off under VoiceOver Recognition and sure enough, it disappeared from Quick Settings. 

When I turned it back on in VoiceOver Recognition, I had to reboot the phone before it showed up again in quick Settings.  Not sure why, but that is what I did.

 

My recommendation is don’t have it in the rotor, but just in quick settings.  There are other settings you may find it better to have in Quick Settings instead of in the Rotor. 

 

Once again to review, quick Settings is launched with a 2 finger quadruple tap.  It functions like the rotor, but since you have to be more intentional about bringing it up, you won’t find yourself changing voice speaking rate when you wanted to jump by headings, etc.

 

You can also enable Screen Recognition for specific apps once you figure out it helps in that app by going into VoiceOver Recognition, Screen Recognition and double tapping on apps, and selecting the apps for which you want it to default to on.
