I think also in the receipt you hopefully get via Email, or on the web site 
under your account, there is a return link I think.
It has been a very long time since I purchased an app that I wanted a refund 
for.

But, calling Apple Accessibility should get you on the right track.
You can always challenge the charge through your credit card company if nothing 
else works.
