Jed, unless you list the exact model of smart TV you have nobody can tell you 
what you need. The fact is that most smart TV's have apps for the major 
streaming services preinstalled, e.g. Netflix, Disney Plus, Apple TV, Prime 
Video etc. Furthermore most smart TV's have their own app store where you can 
download additional apps if they are available for the platform your TV uses. 
The most common ones would be Roku, Web OS (that is LG) and Google TV (Sony and 
many others). Also most new smart TV's have some sort of screenreader built in, 
but how well it works you would have to try and I would venture a claim which 
is that most likely the Apple TV box with Voiceover probably is still the best 
experience for a blind person.
We have one of the higher-end LG OLED TV's, the C-series although ours is from 
2 years ago and there are new ones this year, but we also still have just 
bought a new Applt TV 4K, also the one which was released this year. Partly we 
also do this because it acts as a hub for Apple Home Kit and if you buy the 128 
Gb version which I believe is $149.99 in the US you get a device which also has 
an ethernet port and is capable of thread networking which may or may not 
become important in the years to come. This is a protocol used by the new 
"Matter" smart home standard and Apple, Google and I believe Amazon have all 
signed on and said they will support it. Basically if you have a thread network 
capable device such as an Apple TV or a router (many of the new routers are 
also thread network enabled) then connected home devices can talk directly to 
each other without having to go out over WiFi.
This is of course just a simple explanation, if you want to know more you can 
Google it as there are many articles available about Matter and thread 
networking.
Also, if you, for example, have a Sonos Arc which can play Dolby Atmos and you 
want to take advantage of Apple Music songs which are encoded for Dolby Atmos 
(special audio) you have to play them on your Apple TV which is connected to 
the Sonos Arc soundbar via the TV. You can play Apple Music directly from the 
Sonos app, but at least at this point you don't get special audio that way.

Hope all is well.  Hoping some of you can help.  I have a super old apple tv.
Now, I ahve a new smart tv.  Do I need a physical apple tv any more, or is 
there just an app I would install?
I'm so far out of the loop when it comes to apple tv.  1 of the things I have 
watched a lot os pluto tv.  Would I need the pluto tv app, or would I just 
access it all through the apple tv app?  If someone can help, that would be 
awesome.
