As I said, turn off the automatic switching and see if that helps: Settings > Accessibility > Voiceover > Audio, turn of "Auto Select Speaker in Call" If this fixes your issue then it is quite likely that your phone's proximity sensor might not work correctly which then would not be a Voiceover bug.
