As I said, turn off the automatic switching and see if that helps:

Settings > Accessibility > Voiceover > Audio, turn of "Auto Select Speaker in 
Call"
If this fixes your issue then it is quite likely that your phone's proximity 
sensor might not work correctly which then would not be a Voiceover bug.

The strange thing is that, after first answering a call, I pull the phone away 
from my ear, so it will go to speaker.  After about a minute or less, the 
speaker cuts out and I have to bring it back to my ear to hear.  Then, again,

This happens every thirty seconds, or so.  Also, a couple of times, while 
holding the phone to my ear, the speaker will kick in again, then go back to
