If focus modes or "Do not disturb” are enabled then some or all calls can be 
directed to voice mail. You can  usually test this by having the person who is 
going to voice mail try calling you again almost right away and then seeing if 
it rings. 
It is possible to disable this break through on second call attempt but the 
default  in most focus modes is to allow break throughs.
Sometimes when I have a meeting scheduled on my iPhone calendar, there will be 
a prompt on the lock screen asking if I want to enable do not disturb for the 
meeting. One can also enable/disable focus modes with SIRI.
