Once again echoing Richard, can you specifically check and answer whether screen recognition is on? Turn the rotor until you hear screen recognition, if it says "on" flick up or down with one finger, then make sure you turn the rotor clockwise again to something like characters or words, if you leave it on screen recognition it's just too easy to turn it on again. I have an iPhone 13 Pro and use iOS 17 and all is fine in the Health app.
