Direct touch allows apps to provide a part of the screen where gestures aren't captured by voiceOver. Think of self voicing or braille keyboard aps. The direct touch setting allows this to be turned on or off for the app.
Screen recognition gives you access to app controls that may not be visible to VoiceOver. This will make some apps more usable. If the two finger double tap doesn't hang up a call then you may need to wait a beat after you pull it away from your ear before using the magic tap.
