Don't US carriers offer a feature which requires a caller to press a random 
number before the call is connected? Here in Canada you can ask for this and if 
for example I call somebody who has this activated, I hear the following 
message:

The subscriber you are calling has call control. Please press 5 to be connected.

Of course the "5" is just an example, it could be any number, like I said, it's 
random and a different one each time you call, it could be 1, 8, 3 etc. The 
point is that unless you press the correct number you will not be connected. 
Only thing which could be an issue is when you get a call from a legitimate 
automatic system. For example, sometimes a health care provider may have an 
automated system calling you to remind you of an appointment and in some cases 
you may have to press 1 to confirm the appointment. Otherwise this seems to be 
very effective. I don't personally have this activated on my phone because I 
don't actually get many such calls, maybe 3 or 4 a year and in those cases I 
just hang up once I realize it's not a legitimate call.

This might not be what you want to hear, but I am also a T-Mobile user, and 
never found Scam Shield all that great. So what I do is the nuclear approach, 
which means I have set the setting in my iPhone so that the only calls that 
ring through are those in my contacts. I get notifications if there’s a 
voicemail, And the bad side of this of course is that if you’re getting a call 
from somebody that’s not in your contacts, you’re going to have to call them 
back. So that’s the downside. The upside is I don’t get spam calls on my phone 
ever. It’s worth it to me.
