This sounds like Audio Ducking is kicking in when not wanted. You can add Audio Ducking to your rotor (Settings,  Accessibility,  Rotor Items). Much of the time I’m listening to or watching something, I’m not using my phone for other things, so VoiceOver is left alone, and I’m honestly baffled as to why my phone is doing this. I don’t know if it has to do with an update, but I doubt it. Any ideas?
