After I expand the thread, I swipe down to the first message and let VoiceOver speak the message without actually opening the message which I only do if the message is too long to be spoken by my preview setting. I don’t have previews turned on. Also, I never expand the thread and swipe down to the first message as you described. I just double-tap on the thread and, it it’s a brand-new thread, I assume that I’ll land in message 1, read it and go on from there.
