Yes, it does indeed work with the always setting.  But you have to have some 
other media playing to notice it.

If you have the volume at 199, the media will be almost silent when VO is 
speaking.

Adjust accordingly.

Audio ducking absolutely works while in the always mode. I've come across 
messages suggesting that it's buggy in the "only when speaking" mode. Anyway, 
if you set ducking to always and then adjust the volume in the rotor or 
VoiceOver settings or quick settings, you can move it above 100 percent. The 
higher you set it, the more other audio will be reduced in volume to compensate.

Good apps to test this with are the CBC Listen app here in Canada. It's louder 
than VoiceOver unless you use audio ducking. It works nicely with Apple Music 
as well. You can get VoiceOver to be comfortably louder than the music and yet 
still be able toenjoy both things.
