I think it's worth the US$20 I paid. I wrote a detailed explanation of my reasoning on this list a couple of days ago, and I also put that review in the app store, so I imagine it's available there by now.

AccessNote is designed to work with VoiceOver, so yes, it's fully compatible 
with it.
