You are using a two finger double tap. This command automatically starts and 
stops music when not answering / ending phone calls or starting / stopping 
another audio stream.

If you believe you are issuing a different command, go to settings, general, 
accessibility, voice over, practice area and touch the middle of the screen. 
Now, practice the commands until VO announces what you believe you are doing.
