I too prefer MapQuest.  It gives highly accurate voice guidance while
driving or walking, and works in the lock screen or while using other apps.
It even works while I'm on the phone, giving me directions through the ear
piece.  It's more accessible than Google Maps as well.

Jim, it's been a long while since I have used Google Maps, but I believe you
have to make sure that the driving directions option is checked before the
app will talk.  I wish I could explain the disappearance of the "Action
Results" button.  I have found that button essential for speech.  Like
David, I have had accessibility issues with Google Maps.  I switched to
MapQuest and find it generally works well.  The buttons are labeled with
understandable indicators.
MotionX and Navigon also get high marks for accessibility.
