I wonder if my suggestion below would help those who are still having the problem.
Make sure that the setting to change the volume with the buttons is turned on. without VO speaking, press the volume down button repeatedly until you hear no more sound. You may not be able to hear VO at this point when you touch the screen. Do whatever you must to make VO speak again and while VO is speaking repeatedly press the volume up button to the desired level. You may at this point still need to increase the other sound volume using the volume buttons.
