Have you tried a different book? I have a Kindle book that contains sections at 
the ends of chapters which I can't access with VoiceOver. I asked about it on 
this list a week or so ago, but the problem is specific to that book. 
Apparently, there are arrows to be clicked on, but VoiceOver can't see them.
