Do a three-finger vertical flick to bring the add to cart button onto the 
visible display. Find the button with one finger by moving the finger on the 
screen to the button. Whilst holding the finger on the button, touch the screen 
with another finger (split-tap) to activate the button. This works very well. I 
just purchased multiple items with the amazon app today.
