We have had several occurances where we develop new code(various
solutions) commit the changes. Then another developer updates their
version, tests the changes and all is well.

Then at a later time a bug arises, we check and the code is a previous
version prior to our changes.  There is also no log entry as to the
changes ever being made.

Has anyone ever run into this before? If so what was done to resolve
the issue?

We are running Visual Studio 2008, VisualSVN Server version 2.08,
VisualSVN Client version 1.7.6, TortoiseSVN version 1.6.6.17493
