Now I'm totally unsure of what the right thing to do is.

Tell you what.  Grab a copy of the current MakeMaker, look through
ExtUtils::MM_Unix::init_INSTALL and make it work on VMS with PREFIX.
Or tell me what I need to do to make it work.
