You need IP connectivity first.  Then connectivity on ports 5800 & 5900(if
it's a windows host, otherwise 5800 + session number or 5900 + session
number).

Are you using Dialup Networking to connect with the modem?  Can you ping
that server?  Are you using the server name or IP address?(you might need to
use it's IP address, ie '192.168.0.2:0', in VNCviewer)  If you can ping that
host, then you have to look for a firewall or something like it restricting
those port numbers.

VNC works fine by me to connect to any server on the lan (all machines under
Windows 98). But when I try to connect to a remote server through my modem,
