OK, you need to click the window menu (the little icon at the top left hand
corner of the VNC viewer window - the one you would double-click if you
wanted to close the window), on there you will find, among other things, an
option to send ctrl-alt-del.
Of course if you are using NT and you want to log-on remotely, don't forget
to set the default password.
