I really wish Xvnc would use the same startup files as XFree86 does, then this kind of 
thing wouldn't happen.  Assuming you're using a Red Hat (or derivative) distribution, 
symlinking ~/.vnc/xstartup to ~/.Xclients works for me:

[chromi@beryllium chromi]$ ls -al .vnc
total 12
drwxr-xr-x    2 chromi   chromi       4096 Apr 25 01:21 .
drwx------   30 chromi   chromi       4096 Apr 24 04:59 ..

[chromi@beryllium chromi]$ cat .Xclients

    exec $HOME/.Xclients-$DISPLAY
else
    exec $HOME/.Xclients-default
fi
[chromi@beryllium chromi]$ cat .Xclients-default
# Created by Red Hat Desktop Switcher
exec startkde
[chromi@beryllium chromi]$

This is with Red Hat 6.2 and KDE 1.1.2 (a little out of date).
