Could you share a copy of the attributes you setup on the accoount login?
Qwest appears to only be sending username and password, when i turned on debug.

Sometimes Qwest likes to try to authenticate with login-user as the service
type instead of framed-user. You can either set your raidus server to
default PPP for every service type, or tell Qwest to stop being jackasses
and authenticate how they are supposed to authenticate a dialup session.
Myself, I made them fix their end, as I use login-service for login-service
type stuff. If they don't get it, turn your debug logging all the way up,
and show them their request and a normal users request.
