Happens all the time.  I think it's because MUI needs the program to tell it
to close the window, and since the program has had an error or crashed, it
can't tell MUI to do that.  It happens with ImageFX too, sometimes, and I
think ImageFX must have a similar window management setup to MUI, even
though it is not an MUI program AFAIK.

Nothing you did, don't worry about it too much.  If you want, you can
usually use one of those realtime manager programs to force the window
closed.  But it's not likely you will get Voyager running again without a
reboot, so there's not much point to that.
