With respect to question 3 in your post, I can tell you that split
tunneling can be disabled in the client configuration, but the control
is exposed to user manipulation.
Split tunneling is best denied by the configuration on the VPN gateway
to prevent users from changing the client settings.
I'm not authoritative to answer questions 1, 2 and 4.
