Personally I do not have access to a Mac machine. I made a few tests with
version 1.5.2 on an IMac and I was able to start the thing but I didn't do
extensive testing. Is anyone interested in helping vTcl for Mac ? Any help
welcome!
As for people not using the GUI development tool and going directly to Tk,
it's probably because they get more flexibility in hand-writing the code
themselves, and more security as well, since a GUI builder (vTcl included)
can corrupt the whole project because of a bug (don't forget vTcl always
makes a backup copy, though, so you can recuperate from crashes).
Also, a project like vTcl maintainted by only one active developer cannot
reach the stability and feature richness of software like Visual Basic
where a team of more than 100 programmers has been working for years. A lot
of work has been done, in particular a lot of megawidgets are now
supported, but a lot remains to be done as well, which is why there aren't
many releases. Documentation is a problem, too, since programmers do not
like to waste time documenting :-)
Sometimes people feel discouraged by bugs and don't use the product at all
instead of following up. I think this offen happens with free software. If
the first impression with a software does not work well, users will switch
to a different one or not use it.

Do you get an error message? If so, letting us know what it is would
help.
Note that most Tcl/Tk developers from whom I've read messages tend to use
Xf , vtcl, or SpecTcl a few times, then move over to writing Tcl/Tk
directly.
Whether it is that Tk is so easy to write, or the tools leave something
to be desired (either from documentation or functionality) I'm uncertain.
