I'm really sorry but our product is not good at converting word files
which mainly contain forms. W2x has been designed to convert word files
which mainly contain *prose* (paragraphs, lists, tables, pictures,
headings, etc).
Note that it is probably possible to create word documents in which
(once filled) form data are automatically saved by Word into a separate
XML file stored in the .DOCX file (which is basically a Zip archive).

If you manage to do this,
1) there is not need to purchase any special product. Hire an XSLT
programmer to write a simple script (first step: unzip form data from
the DOCX file, second step: convert form data to a <Report> using an
XSLT "stylesheet");
2) the accuracy of the result you'll get is guaranteed as it is Word
itself which does most of the job .
