Sometimes Word does not recognise the dictionary if the "Language" of the document is not what you originally set it to. I set up Word with my language as English AUS. All the stuff added to my user dictionary is available only when the Language is English AUS. If I open a document with the language set as something different (English US for example) my user dictionary is not available.

The way round this is to change the "Language" in the document. Select all the text in the document (Command + A). Then go to the Tools > Language > English AUS menu item. If you change the language to your original one then the user dictionary should now be available.
