Try isolating ~/library/caches/com.apple.help.ui (or wherever it's
located in 10.4.x)
Getting rid of those caches works in 10.3.9

Nancy's suggestion of trashing ~/Library/Caches/com.apple.helpui is
correct.
Close and re-open Help Viwer, and the proper contents should be
displayed.
Failing the above solution, you may need to delete the following
property list files from the ~/Library/Preferences folder.
com.apple.helpviewer.plist
com.apple.help.plist
com.apple.helpui.plist
and re-launch Help Viewer.
