I don't have personal experience here - but I have been following the thread with interest. >From Rob's and Stephen' emails it sounds like they had similar problems and the solution was to delete the device from your iCloud account settings. You never actually confirmed that you had done this? I would imagine that these settings are stored on the iCloud server and associated with your iCloud account and Apple ID and the specific device(s) you have connected. If these settings are somehow corrupted, I imagine that deleting the device clears this particlar area of the settings and then recreates them when you re-connect the phone to iCloud. (A bit like when Ronni suggests people delete a specific plist file to clear a persistent problem with some app or setting?) If this is the case then even completely reseting your phone back to factory settings may not help you. When you go to set-up the phone again and, in particular, setup your iCloud account, you will connect to the iCloud servers and, if the problem is with the settings held there for that device, which I think it will still recognise, it may still ask you for your activ8 password. Of course, this is pure speculation on my part and if you have already gone through the steps Stephen outlined and successfully deleted the phone from iCloud then please ignore this. If not, I would definitely make sure you first tried deleting the phone from your list of iCloud devices first. Stephen suggested the steps he remembered and Rob provided a link to the relevant Apple support page: <https://support.apple.com/en-au/HT205064> which clearly explains how to remove a device from the list. If this doesn't fix it and you do need erase the phone to restore to factory settings, then this Apple support page: <https://support.apple.com/en-au/HT201274> clearly walks you through it.
