Why should people who don't use Microsoft be forced to download special utilities just to read this form? Pdf requires a special viewer as well. Since this is a web site use web technology. Html and css create perfectly usable forms. When properly styled the forms will automatically shift to accommodate printer settings. And the forms can be written to handle 508 issues as well.
