It did helped for 2+3 issues (for sure for 2),
But I'm still with MySQL logging.
/var/lib/mysql/mysqld.lod showing me only start/stop info on mysql.
I want to see log interaction with mysql when user authenticate itself.
Is there loglevel variable to change so it would enabled?

Another interesting thing: /var/log/web-cyradm.log is empty even though it's 
owned by wwwrun.

To solve 2+3: Add in /etc/pam.d/imap

sqllog=yes
(in the line with auth sufficient..)
This is requiered for  the latest versions of pam_mysql
