I unfortunately help because Google Groups is not showing your code formatting very well (nothing is indented). Multiple forms on one page is definitely challenging especially since I think you want one form to be based on the other.
We might need to see more of your code (where's the "return"? maybe the view, too?) and how it's formatted.
